1.35pm-1.50pm: LGT Update; primary/secondary care interface work
1.50-2.15pm: Safe Surgeries session with Katya Griffin, Lewisham Borough of Sanctuary Programme Manager
2.15pm-2.50pm: Setting the scene with Rebecca Corneck, Kiera Doherty and Kathryn Griffiths
- What are integrated neighbourhood teams (INT)?
- Are there any examples of integrated neighbourhood team working through the lens of multiple long-term conditions (e.g. cardiorenal metabolic diseases)?

1. How do I unblock barriers to achieve more successful engagement (CKD lens)
2. Recognition of the challenges / elephant in the room
3. Specific CRM clinical education – How can I provide high quality mLTC care (through a CRM lens)?
