This session was to be an informal chat to look at how to easily incorporate conversations about condoms into your everyday conversations with patients. Due to low take up of bookings, the decision was made to cancel the L&L session.
This session will discuss who you can offer them to (almost everyone), how to record on EMIS, how to manage condom collection via the front desk, any info you need to pass onto reception staff to do this, and how to do a speedy condom teach. Oh and how to order more! If there is time left at the end Sheila will be happy to chat through any STI/contraception conundrums that you may have.

This session covered what a severe asthma referral looks like, an introduction to the SE London Severe Asthma team and some tools that will be available in the New Year to start to ID some patients.
Asthma is not evenly spread, with higher rates in: African, Caribbean and Asian families, People living in deprivation and/or close to major roads. There are over 20 asthma deaths across SEL every year, including adults and children and young people. Asthma is the 3rd most prevalent condition in South East London. The session also provided some background as to where SE London is currently and why some of the asthma changes are taking place. Grainne also offered support around Identifying and referring the at risk Asthma patient.

NB Medical provided you a live fully interative Hot Topics Update. This online learning experience included questions and taking part in polls, making the whole experience as exciting and as fulfilling as attending a face to face course. The “Hot Topics” course consisted of a series of short ‘TED-style’ engaging presentations blending evidence and cases, followed by discussion which may have an impact on practice. The course comes with a number of extra resources to give it incredible ‘added value’. Those unable to attend the live session would be able to receive a recording to watch in full after the course.
There were 300 spaces available for the Hot Topic Update, offered on a first come/first serve basis. Prior to the event, attendees received a course reference booklet posted to their practice.
Access to the webinar was via the NB Dashboard. Attendees received an email from NB Medical with their link and login details.

The Urgent Community Response (UCR) team provides a community rapid response to Lewisham borough residents seven days a week, Monday to Sunday from 8 am to 8 pm. This service treats patients at home to avoid hospital admission or readmission. All referrals are triaged by the on-call Advanced Clinical Practitioner (ACP) and if clinically necessary seen within two hours (patients will also be seen later that day or the next day if required) by a multidisciplinary team of ACPs with support from consultants, the hospital frailty team, and colleagues in emergency care.
Alongside Nina, Andrew Cook, Virtual Ward Delivery Manager, Lewisham Local Care Partnership, South East London Integrated Care System also provided an explanation on what the Virtual Ward is and how it differs from UCR.

Preventing and treating diabetic foot ulceration is a key focus of the work of any NHS podiatry service. For the purposes of NICE Guidelines (Diabetic foot problems: prevention and management) the Lewisham Foot Health Service is the Foot Protection Service for patients with diabetes, with primary podiatric care linked to the Acute Foot Service based in UHL and with access to the multi-disciplinary foot team. The presentation covered the risk factors for diabetic foot ulceration, the principles of managing and treating foot ulceration according to current guidelines and prevention of diabetic foot problems.

The Lewisham Foot Health Service provides a wide range of foot health services at University Hospital Lewisham and in a number of community clinics across Lewisham. This includes assessment, diagnosis, treatment, education and advice on disease and deformities related to the foot and lower limb. Patients may present in primary care with a variety of foot complaints, from nail pathologies, skin lesions and musculoskeletal disorders to foot ulceration. The presentation ran through the types of clinics we offer and referral pathways into the foot health service for different types of conditions, how to support patients not eligible for ongoing care by the NHS podiatry team, including where to find information to help patients to self-care.

Interstitial lung diseases (ILD) are a heterogenous and complex group of disorders. The fibrotic ILDs are progressive with severe impacts on quality of life and mortality. In recent years the advent of anti-fibrotic therapy has triggered the centralisation and specialisation of ILD practice. However, as the license for these drugs broadens to include all progressive fibrotic lung diseases, at wider ranges of presenting lung function, and as more patients are referred in due to increased incidental detection through programmes such as the Targeted Lung Health Check, the pressure on these central and specialist clinics is not manageable through increased capacity in those services alone. BTS and NICE both now encourage the development of capacity in secondary care and local services to manage these patients. We will present and discuss the service at Lewisham Hospital, which has been at the forefront of the developing Shared Care model.

It will provide Private Fostering Awareness and is suitable for any professionals who come into contact with children during the course of their work. They should be aware of what private fostering is, why this knowledge is important, and their responsibility in the reporting of such cases. An example of a Private Fostering Placement was Victoria Climbié. The Aims and Objectives for the session is to Provide an overview of professionals duties and responsibilities in safeguarding privately fostered children and young people / Explore legal definitions and the legal framework of Private Fostering / Simple identifying indicators of private fostering arrangements / Explore a range of issues that can overlap private fostering, such as child trafficking and exploitation / An opportunity to share and explore best practice.

This session was to be facilitated by Dr Esther Appleby, GP working with the Learning Disability and Autism Programme SEL.
People with learning disabilities are 10 time more likely to have a serious eye problem than the rest of the population and yet much less likely to get the eye care they need. This results in significant incidence of avoidable sight loss – both reversible and irreversible. SeeAbility campaign for an ‘Equal Right to Sight‘ for people with learning disabilities. Lisa Donaldson has led SeeAbility’s clinical research in special schools which contributed to NHS England committing to a rollout of eye care services in all special schools. Lisa was to present the barriers to good eye care and vision for people with a learning disability and possible solutions but this session had to be cancelled. It is hoped it will be on the June PLT agenda.

When the heart isn’t pumping blood as well as it should, it means the body is not getting enough oxygen which affects how it should work, including breathing and muscles, in turn, causing some of the main symptoms of heart failure. This session covered ‘What is heart failure’, the Symptoms to look for, it’s Causes, the Burden of heart failure and what Investigations may be carried out to help manage the symptoms and improve quality of life.

Autism is a neurodevelopmental condition which affects around 1% of the population. There are approximately 4000 patients with a formal diagnosis in Lewisham and another 600 on a waiting list. This session presented an approach to assessment, management in primary care, and when a referral is required. Life expectancy for the autistic population is approximately 16 years lower than the general population. In this update, we explored the reasons behind this and what you can do in primary care to reduce health inequalities. The session briefly outlined the wide spectrum of presentations in ASC, when to suspect autism, referral pathways, current waiting lists and the support services available in the community.

This session provided a brief overview of the Dynamics of Domestic Abuse, the Lewisham picture of Domestic Abuse and its impact on patients. In response to the Lewisham VAWG Strategy 2021-26, Zuzana identified how we can best contribute to the 5 key priority areas identified; Prevent, Protect, Recover, Pursue, Partnership. As professionals on the frontline how does this influence our approach, what are the restrictions we face and what support is available? This session provided an opportunity to discuss and share useful tips, information and suggestions for change.

In response to the issues raised, a steering group was set up with SEL GPs, digital leads and commissioners. Through this group, the current arrangements were discussed in detail, followed by formulating and agreeing on the desired functionality from a SEL repository system. We are keen to gain feedback from our primary care colleagues on the suggested changes that we are looking to make going forward and this session will be an ideal opportunity to achieve this.
And the second, Virtual Ward Update; Virtual wards allow patients to receive the care they need at home, including in care homes, safely and conveniently rather than in hospital. They also provide systems with a significant opportunity to narrow the gap between demand and capacity for secondary care beds, by providing an alternative to admission and/or early discharge. There are four key work streams that form our Virtual Ward programme here in Lewisham; Remote Monitoring, Admission Avoidance, Urgent Community Response and Early Discharge. This session provided updates on two key areas; Admission Avoidance and Early Discharge.
